___________________________________________ (buyer) and is entered on this date: _____________________________________________.
Pertaining to:
Puppy Identifier: ____________________________________________________________
Born on: _______________________________________
Sire Name: _____________________________________
Dame Name: ____________________________________
This contract must be signed by the puppy's new owner (owner name) __________________ and returned to the breeder (Doodle-Love Labradoodles/Kristen Fuhrmann), no later than (7) business days of the date noted on this contract, which is also the date of sale/transfer of the above referenced puppy. Thereby, if this contract has not been signed and returned by (date)_____________ which is (7) business days from the date that the puppy was sold/transfered to his/her new family, this contract is forfeited by the buyer. In the case of the contract not being signed and returned - the puppy's health guarantee, registration, and access to pedigree is forfeited by the buyer.
Seller's Responsibility:
1. The Seller guarantees the puppy is in good health and has been de-wormed (puppies are on de-worming programs starting at 3 weeks)
at the time the Buyer takes possession. The Buyer has three business days to take the puppy to a veterinarian for an examination. If the puppy is found to be in poor health the Buyer has the option of returning the puppy to the Seller, at Buyer’s cost, for a full refund or keeping the puppy. If the puppy is not taken to a licensed veterinarian within the three business days, the Healthy Warranty is void. (Documentation is required from the licensed veterinarian office of your choice stating that the puppy has been examined by a licensed veterinarian within three business days of receiving the puppy. This documentation is required to be returned to the breeder no later than (7) business days of the exam to keep health guarantee valid)
2. The Seller provides this warranty for two years after the date of sale against any debilitating congenital defects. In the event the puppy exhibits symptoms of a suspected debilitating congenital defect, the Buyer must supply any requested veterinary records to the Seller.
Eye Defects: This requires an exam by a certified Canine Ophthalmologist and a signed statement of
diagnosis as well as all medical documentation from date of purchase to current.
Hip Defects: *· An evaluation of hips must be completed by OFA when the puppy is 18-24 months of age for a puppy to be returned for hip dysplasia. This will be done at the Purchaser’s expense. If the Puppy is
diagnosed by OFA as being severely dysplasic on or before 24 months of age, the Purchaser must submit a copy of the OFA report to the Breeder. A veterinarian cannot make the diagnosis, it can only be made by
OFA accredited hip specialist certifiers and the Puppy must be sedated for the radiograph to be taken.
In the event of a genetic defect covered by this guarantee, a detailed letter from the Purchaser’s veterinarian describing the exact nature of the illness or defect must be presented for replacement. Evaluation of the hips by OFA with results of Fair, Good or Excellent are recognized as normal hips and an OFA number will be assigned. These ratings are recognized as healthy hips and will not be eligible for a replacement puppy.
The purchaser has several options if the puppy/dog has a genetic defect (severe hip dysplasia or eye defect).
a. If the puppy/dog does need to be laid to rest because of the severity of the defect, the buyer may
choose to receive a replacement puppy of the same value (f1 for f1, f1b for f1b) within 1 year of defect
diagnosis.
b. If the purchaser chooses to keep the puppy/dog and keep up with the condition:
The purchaser can also choose to have a replacement puppy (purchaser will be responsible for shipping cost of the puppy).
We do not require the defective dog be returned to us. We know you love and care for the dog and
wouldn’t expect you to part with it at a time when it needs you.
Purchasers will be required to show to the Breeder veterinary receipts and medical records as indicated
above. Breeder reserves the right to have an additional examination by the veterinary clinic of our choice
prior to providing a new puppy.
3. This warranty does not include viral illnesses, infections, improper bites, hernias, hypoglycemia, Guardia or Coccidosis. It also does
not include any illness due to the ingestion of foreign objects, food, chemicals or physical injury.
4. If it is determined the puppy does have a life threatening genetic defect that is identified within two years of purchase, the Buyer has the option of receiving a replacement puppy from the next available litter. Your puppy need not be returned to us. Shipping fees will not be refunded, and if it becomes necessary to ship a replacement puppy, those fees are to be paid by the Buyer.
5. If the puppy should die from a suspected congenital defect within the 24 months of its date of purchase, an autopsy must be performed at the purchaser’s expense by a veterinary teaching hospital or state licensed DVM acceptable to the Seller’s own veterinarian. If the autopsy shows a congenital defect as the reason or the death, the Seller will provide another puppy of equal value to the purchaser. A written statement from the veterinarian must be provided. This warranty is void in the event of an indeterminable or inconclusive diagnosis.
6. Seller agrees to provide a pedigree and number of the microchip that has been implanted in the puppy, if applicable.
7. No warranties or guarantees, expressed or implied, are made under its contract except as stated.
8. This warranty is non-transferable. If the puppy is sold, or rehomed, this contract is void with the new owner. This contract only applies to the origional purchaser of the Labradoodle.
